VA Benefits & Forms for Veterans

Important forms and resources for Veterans seeking caregiver support and benefits

    Honoring Those Who Served

Veterans have access to several programs that can help cover the cost of in-home care. Below you'll find the specific forms and programs available, along with guidance on how to apply.

    1. Caregiver Support Benefits

If you are applying for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), which offers a monthly stipend and access to health care for caregivers, you need this form:

    VA Form 10-10CG

Title: Application for Comprehensive Assistance for Family Caregivers Program

Purpose: Used by the Veteran and the primary/secondary caregivers to apply for the program. Both the Veteran and the caregiver must sign this form.

    Download Form 10-10CG

Note: For the general support program (PGCSS), there is no specific application form; you simply need to be enrolled in VA health care and request a referral from a facility Caregiver Support Coordinator.

    2. Directive Veteran Care

There are two distinct concepts this might refer to. The most common form requested regarding "directives" is the Advance Directive, but there is also a program called "Veteran Directed Care."

    A. Living Will / Medical Power of Attorney

    VA Form 10-0137

Title: VA Advance Directive: Durable Power of Attorney for Health Care and Living Will

Purpose: This legal document lets the Veteran state their treatment preferences and name a specific person (Health Care Agent) to make medical decisions for them if they become unable to do so.

    B. "Veteran Directed Care" Program (VDC)

    No Single Form Required

This is a specific program that allows Veterans to manage their own budget for home and community-based services.

How to Apply: There is no single "application form" you download. You must contact your VA social worker or case manager to assess eligibility and begin the enrollment process.

    3. Aid and Attendance (A&A) Benefits

Aid and Attendance is an add-on benefit paid in addition to a VA Pension or Disability Compensation. You cannot apply for it entirely on its own; it must be attached to an existing or new claim for pension or compensation.

    VA Form 21-2680 (Primary Form)

Title: Examination for Housebound Status or Permanent Need for Regular Aid and Attendance

Purpose: This medical form must be filled out by a doctor (MD, DO, Physician Assistant, or Nurse Practitioner). It documents the Veteran's physical limitations to prove they need daily assistance from another person.

    Download Form 21-2680

    VA Form 21-0779 (If applicable)

Title: Request for Nursing Home Information in Connection with Claim for Aid and Attendance

Purpose: Use this specific form only if the Veteran is currently living in a nursing home. It serves as proof of nursing home status to fast-track the A&A claim.
